    Overview

    Continue CareRx Pharmacy provides comprehensive care services, including pharmacy support, virtual nursing, and coordination of healthcare resources. Our team is dedicated to ensuring patients receive the right care at the right time.

    The Enrollment Coordinator works under the supervision of the Continue CareRx Director of Operations. Key responsibilities include assessing patient eligibility, providing education, and enrolling patients into the Continue CareRx Pharmacy. In-home visits are conducted as needed.

    Qualified applicants must possess:

    • Pharmacy Technician - Nationally Certified or State Certified

    • LPN, LPT, LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice

    • Licensed Social Worker with a degree from an accredited college and current unrestricted registration and license in the state of practice

    Schedule: Monday - Friday 9am - 5:30pm ET

    This position is based in the PharMerica Indianapolis, IN pharmacy. Applicants must live in or be able to commute to Indianapolis.

    Travel: 10%


    Responsibilities

    • Proactively contact eligible patients to introduce and explain the Continue CareRx Pharmacy Program, ensuring they understand its benefits and assisting with their enrollment
    • Address patient inquiries, provide program-specific information, and ensure timely follow-up to ensure optimal patient satisfaction
    • Maintain accurate records of patient interactions and enrollment statuses within the system, ensuring all data is up-to-date and compliant with company standards
    • Participate in monthly Branch Director meetings to provide insights, report on program performance, and offer feedback for improvement
    • Monitor and track key program metrics (e.g., enrollment numbers, retention rates), identifying trends and ensuring program goals are being met
    • Report on outcomes and suggest initiatives for enhancing program effectiveness, collaborating with leadership on strategic improvements
    • Serve as the primary point of contact between branch operations, program leadership, and field clinicians, ensuring clear communication and alignment on program goals and patient care
    • Assist with staff meetings, case conferences, and other relevant discussions where program input or patient care insights are needed
    • Provide field clinicians with guidance and support, helping to troubleshoot challenges related to patient enrollment and engagement
    • Conduct in-home patient visits to facilitate the enrollment process where necessary, ensuring that patients who require additional support receive a personalized experience
    • Collaborate with field teams to coordinate visits, ensuring a smooth and efficient patient onboarding process
    • Deliver outstanding customer service to both internal (branch leadership, clinicians, staff) and external (patients, families) customers in alignment with PharMerica's values
    • Handle any patient concerns or issues with professionalism and care, escalating to management when appropriate
    • Perform other tasks as assigned by management, including special projects, as needed to support branch and program success

    Qualifications

    • LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice OR
    • Nationally certified Pharmacy technician in the state of practice OR
    • Licensed Social worker with a degree from an accredited college with current unrestricted registration and license in the state of practice
    • Valid driver's license, acceptable driving record and proof of car insurance
    • 1+ years of medical related experience preferred
    • Home health care experience preferred but not necessary
    • Able to bend, stoop, push, pull, sit, and walk for periods of time
    • Detail oriented, goal oriented, team player, customer service, excellent oral communication
    • Demonstrates a strong commitment to providing exceptional customer service to both internal and external stakeholders, including patients and their families. Must be patient, empathetic, and capable of building positive relationships.
    • Proven ability to work effectively with cross-functional teams, including leadership, clinicians, and administrative staff, to ensure high-quality patient care and program success.
    • Ability to manage multiple priorities while maintaining accuracy and adhering to deadlines. Must be detail-oriented, ensuring all patient data is recorded accurately and promptly.
    • Excellent verbal and written communication skills are essential, particularly in engaging with patients, explaining program details, and liaising with internal teams.
    • Capable of identifying challenges and proactively offering solutions to improve program outcomes and patient satisfaction. Must be adaptable and able to navigate complex situations with professionalism.
