Alopex - powered by Shifox technology is rapidly growing and looking for a self-motivated, experienced RN to work with our Care Management team.

This remote role is a multi-faceted job requiring clinical skill, managerial experience, and customer service and client relationship management.

As the lead contact with their clients, this supervisor position would oversee a care team of coordinators (LPN/MA) and handle customer relations, care planning, and patient triage.

Daily duties include tasks such as: directly onboarding patients to care management programs in accordance with Provider guidelines, completing detailed review of patient records inside EMRs, developing and maintaining individualized Care Plans for patients with acute, chronic and complex diseases, implementing and coordinating all care management activities across the continuum of care, communicating with the patient's clinical care team, monitoring patient progress toward desired outcomes through assessment and evaluation, maintaining accurate and detailed documentation in Shifox software, conducting monthly care calls to patients, addressing follow up needs/concerns for patients on caseload, providing patient/ caregiver education on chronic conditions, reinforcing provider directions related to their conditions, and evaluating and addressing social determinants of health, their effects on the patient outcome, and patient qualification for other care management programs to ensure proper compliance.

The role also includes supervisory tasks such as onboarding.

training, overseeing, and working with assigned care coordinators team to ensure effective care management of their caseloads, supervising and mentoring a teams of LPNs/MAs to ensure productivity and adherence to patient assignments, and communicating with their Provider clients no less than monthly reviewing reports, metrics and patient results while solutioning resolutions to any issues or concerns.
