Job description

Description: The Case Manager is an integral part of Care Net's Support Services team and is responsible for building connection with clients and assisting the general client services team to serve our clients with excellence. The Case Manager role requires strong active listening skills, patience, and problem-solving abilities. This role requires a gracious heart that seeks to love and honor all clients as they are helped through all the life decisions that are part of pregnancy and parenting - and a heart for collaboration and teamwork.

Qualifications/Background:

  • Be aligned with the vision and mission of Care Net Pregnancy Center and be in full agreement with Care Net's Principles, Purpose, and Faith Statement and the Mission and Values Statement
  • Regular attends church and is part of a faith community that aligns with our Statement of Faith
  • Minimum of bachelor's degree, or 3+ years' experience in case management or client services preferred
  • Approved DPS Criminal and CPS Background checks
  • Advanced computer and database management skills
  • Confident communicator and problem solver with exhibited skill in interpersonal communication, ability to manage details, and problem solving
  • A curious mind and ability to problem solve
  • Highly organized, self-motivated, dependable, responsible, and able to work independently and calmly in an often highly emotional environment
  • Availability for occasional night and weekend community events
  • Willingness to provide prayer and spiritual support to clients, if necessary

Responsibilities:

  • Be ready, willing and available to see any clients for all services and build relationships with clients to create a safe, loving, encouraging environment
  • Present overview of support programs to new clients and identify current barriers faced by clients
  • Create 'game plan' and coordinate efforts to help clients overcome obstacles and engage them with case management, educational classes, community partners (when necessary)
  • Assist clients in applying for assistance with Your Texas Benefits, WIC, and other services
  • Provide financial assistance to selected clients who have needs with birth certificates, utilities, IDs, rent, education, and work skills, according to budget
  • Participate in training as required
  • Understand and refer to our to community partners as necessary
  • Make reminder calls & follow up calls to build relationships with clients and encourage them to continue attending our services for community and support
  • Regularly meet required daily/weekly counseling minutes with in-person sessions, phone calls, referral calls, shopping appointments, and any other opportunities to maximize billing
  • Submit timely invoices into Brightworks for all client services and billable interactions
  • Teach classes for the Education Program as needed.
  • Assist with other ministry programs as needed (operations, education, childcare, mobile outreach)
  • Actively participate in all client service meetings, staff meetings, and trainings as well as keep up with community partner emails and referral lists
  • Participate in mobile outreach events as necessary
  • All other duties as requested
