- In this position you will help ensure that client's are consistently receiving medical care, behavioral health services, and access available resources within their communities.
- Contact clients 2 4 times monthly to ensure quality and efficacy of services rendered.
- Will report regularly to leadership to receive support and guidance in providing needed services.
