    Description
    The Advanced Practice Nurse (APNP) provides psychiatric care to consumers served by Journey Mental Health Center (Journey). Treatment is provided on-site in a Journey Mental Health Care Center location and via telepsychiatry in a hybrid model.

    Under the supervision of the Medical Director and in collaboration with other medical staff, the APNP will be responsible for the psychiatric aspects of clinical care within the team, including both initial psychiatric assessments and follow-up visits (or medication reviews).RESPONSIBILITIESThe Journey APNP provides direct psychiatric services to consumers assigned to him or her, including comprehensive evaluation, diagnosis, treatment planning, and treatment.


    This includes:


    Psychiatric assessment, consultation, and/or participation in service planning for individual consumersOngoing monitoring of consumer progress and mental status as indicated in the treatment planPrescribing and monitoring of prescribed medication as determined by clinical need.

    Timely completion of chartingEducation of consumers regarding at-risk behaviors that predispose to medical illnessHealth screening for symptoms indicative of physical diseaseConsultation to treatment team related to consumer presentation, relationship between psychosocial and physical health issues, and appropriate use of psychotropic medications, their side effects, toxicity, etc.

    Work with the program manager or coordinator to support the team functioning and enhance the quality of care provided by the programMedical/psychiatric backup to other prescribers employed by JourneyMedical/psychiatric backup to the Emergency Services UnitParticipation in the training of students and staff, as requestedWork with community caregivers (i.e., primary care providers, outside service providers, housing supports, etc.) to ensure coordination of careParticipation in the multidisciplinary team via team meetings, formal or informal consultation, joint meetings with the consumer, etc., to ensure that consumers receive coordinated, comprehensive careParticipation in administrative duties as assigned by the Medical Director (i.e.
