    PRINCIPAL DUTIES AND RESPONSIBILITIES:

    • Contribute to decision-making, teaching, leadership, and quality improvement activities within the nephrology healthcare team
    • Coordinate patient care, including assessment, planning, intervention, and evaluation for a designated group of hemodialysis patients
    • Provide ongoing education to patients and their families
    • Administer medications as prescribed and document medical justifications
    • Respond to emergencies and ensure proper laboratory procedures are followed
    • Collaborate with the Interdisciplinary Team on patient care plans
    • Coordinate referrals to Social Worker and Registered Dietitian as needed
    • Complete CAP requirements for program advancement

    EDUCATION AND LICENSURE:

    • Graduate of an accredited School of Nursing
    • Current state licensure
    • CPR BLS Certification or successful completion
    • Must meet state practice requirements

    EXPERIENCE AND SKILLS:

    • Key qualities: compassion, support, collaboration, reliability
    • Previous experience as a Registered Nurse (preferred)
    • Experience in chronic/acute hemodialysis (preferred)
    • Passing score on the Ishihara Color Blind Test

    PHYSICAL DEMANDS AND WORKING CONDITIONS:

    • Direct patient care involving heavy lifting and assisting with ambulation
    • Prolonged periods of standing and bending over
    • Possible lifting of equipment and solutions
    • Exposure to infectious materials
    • May involve desk work, computer interaction, and travel to other facilities
