Case Supervisors play a vital role in overseeing and coordinating the provision of healthcare services to clients within our organization. We are looking for someone who can work in a fast paced environment and who is a team player.

Responsibilities

• Supervise clients
  • Initial visits with new clients
  • Staffing existing and newly acquired clients
• Conduct comprehensive assessments of client's medical, social, and cultural needs
• Document changes in client's health and living status

Requirements

• Active driver's license with a clean driving record
• Must have a car
• High school diploma or equivalent
• Bilingual proficiency in English and Spanish
• Must comply with State background screening
• Strong computer, written, and verbal communication skills
