    A Brief OverviewResponsible for providing case management for children under the age of three with intellectual or developmental disabilities or delays.

    Ensures quality services are provided in accordance to all agency or government regulations.

Complete required paperwork and maintain accurate records in department databaseCommunicate professionally and positively with othersSchedule, attend, and facilitate meetings as required by the departmentMaintain ongoing contact with family and providers, taking action as necessaryCoordinate, monitor, and change servicesAssist with the transfer of servicesCoordinate referrals and follow-up as neededFind and secure funding for servicesMaintain productivity standards as required by agency or government regulationsTranslate documents and communications in both English and Spanish as neededQualificationsWhat education or experience would you need?Valid Drivers LicenseHigh school diploma or equivalentBachelor's Degree in a Human Services field or 5 years or more experience working with people with disabilitiesCombination of education and experience may be used to meet the education and work experience requirementsWhat knowledge, skills, or abilities would you need?Ability to communicate clearly in oral and written forms in both English and SpanishAbility to use office equipment such as fax machine, scanner, copier, phoneAbility to work as a teamAnalytical thinking skills to solve routine problems using common senseBasic knowledge in Microsoft Word, Excel, and OutlookBasic knowledge of state regulationsBasic knowledge of technology such as internet browsers, PDF software, database systemsBasic organization and time management skillsBasic typing and data entry skillsNote:
