Job summary
The Housing Stability Advocate's primary role is to work on a collaborative team of Housing Navigators embedded at the Diversion Hub to provide Home visits and wrap-around support services to individuals who have recently transitioned out of an experience of homelessness.They will help these individuals set goals and objectives within a psychosocial rehabilitation framework that allow for successful housing stabilization and retention. The position requires carrying out the mission, vision, and values of City Care by familiarizing yourself with the resources available in the community and establishing a network of contacts to utilize for client care. It also involves visiting clients in their homes and community settings to ensure the development of healthy life skills and facilitate reintegration.,
In addition, they need to develop individualized plans with clients by working together with them identify self-directed goals objectives improve resident outcomes collaborate develop monitor evaluate service plans provide case management assistance resource development crisis intervention direct support residents housing context document progress referrals contacts electronic record update weekly documentation best practices transport clients scheduled appointments connect them necessary community resources serve liaison advocate role between landlords tenants steward landlord relationships other housing providers maximize effective collaboration exceed minimum expectation weekly face-to-face meetings clients embody City Care values People First Whole Care Advocate Overlooked United Collaboration Unrelenting Commitment understanding complexity intersection poverty homelessness mental illness substance use recovery reflect recovery principles minimum three years sobriety skilled written verbal communication ability build genuine rapport quickly multitask stay cool under pressure ability work independently maintain high level accountability willingness transport clients personal vehicles encounter exposure poor hygiene unclean living spaces bodily fluids previous case management recovery justice experience previous experience adults children disabilities ability use computer accurately enter data database HMIS experience preferred pre-employment background check driving record drug test current Oklahoma driver license reliable transportation updated car insurance training familiarity principles de-escalation trauma-informed care Housing First motivational interviewing professional navigating mental physical health systems benefits resources shelter behavioral Health Case Manager Peer Recovery Support Specialist lived environment days hours full-time Mon-Fri
