    TeamHealth is hiring for a full-time medical director role which includes patient care rounding (Monday through Friday) and medical director duties in a skilled nursing facility located in the Seaford/Millsboro, Delaware, area.


    Job Duties:
    Primary care rounding in skilled nursing facilities (SNF)

    Manage chronic disease, diagnosis illness and medical conditions, and implement plan of careCollaborate with clinicians, facility leaders and others regarding care and servicesBuild relationships with your patients, patients' family members, and facility staff to facilitate effective treatment deliveryMedical director duties for assigned facilityProvide oversight, guidance, and mentorship to our cliniciansConduct clinical case reviews, as needed and share tools, information, and guidelines as they relate to quality of careRepresent TeamHealth on appropriate external levels engaging and establishing/maintaining relationships within the post-acute care communityCollaborate with leadership to review care coordination initiatives, develop collaborative intervention plans, and share ideas about network management issuesSkills:

    Excellent clinical skillsCritical thinkingExcellent communication skillsTeam playerStrong work ethicEmpathy, compassion and ability to connect and engage with your senior patients and familyQualifications: 1+ years experience in primary care required.
