- Create personalized care plans for each patient
- Collaborate with the Medical Director to establish policies addressing practice-related issues and emerging health concerns
- Lead team meetings and provide guidance to the clinical staff
- Enhance collaboration with senior living management to ensure effective implementation of care plans and enhance patient outcomes
- Certified Nurse Practitioner with a preference for geriatric experience

The Opportunity
**Pine Park Health** is seeking a **Primary Care Provider - Nurse Practitioner** to deliver compassionate care at Friendship Village. We are looking for an individual excited about working with a dedicated team to offer high-quality care to a select group of patients within a value-based care framework.
