    StarCare Specialty Health System (StarCare) is the state designated Local Authority for mental health and intellectual disabilities for Lubbock, Lynn, Hockley, Cochran and Crosby Counties and is the Local Authority for substance use disorders for 41 counties in the West Texas region. StarCare delivers adult inpatient psychiatric services at Sunrise Canyon Hospital, delivers supports and services to veterans through the VetStar program, and delivers PACE (Program of All Inclusive Care to the Elderly) services to Lubbock's aging population (one of three sites in the state of Texas). StarCare is also a Texas Certified Community Behavioral Health Clinic (CCBHC). StarCare's mission is to provide access to a diverse array of specialty healthcare services that support and empower people as valued members of our community. StarCare has been providing HOPE & HEALTH to our community since 1967.

    StarCare is in search of a Rural Diversion Care Coordinator for Hockley County (Levelland).

    Care Coordination works with individuals in services and the interdisciplinary team to assist in development, management and maintenance of healthcare plans for clients residing within the Levelland City limits. This position will ensure care coordination is implemented by managing patient caseloads, facilitating conversations between Interdisciplinary Care Team members including care coordinators, primary care physicians, and additional providers. Work is performed under the general supervision of the Rural Diversion Team Lead with broad latitude for for the use of initiative and independent judgment.

    Additional responsibilities include:
    • Organizing schedules and managing appointments for patients to ensure they receive services in a timely manner
    • Facilitating communication between the patient, family members, and healthcare providers to ensure patient satisfaction and quality of care.
    • Assisting in identification and application for appropriate benefits.
    • Managing follow up visits and ensuring continuation of care and linkage to community resources.
    • Ensuring StarCare resources are utilized appropriately.
    Pay Incentives:
    Upon completion of 30 days $500Six (6) Month Anniversary$500One (1) Year Anniversary$1,000Two (2) Year Anniversary$2,000

    Schedule: Monday-Friday 8am-5pm with flexibility to meet program needs.

    Qualifications
    • Graduation from an accredited four (4) year college or university with major course work in a related field.
    • Two (2) years full-time experience in a related field, preferred.
    • Must pass a criminal background check and drug screen.
    • Must have current Texas drivers license and be insurable under Center insurance.
    • Must be able to use standard office equipment including laptop computer with Microsoft Office products.
    • Must have strong communication and organizational skills and be able to work within a team as well as independently.
    • Must have a passion for helping people live their best lives.
