    Mennonite Home Communities of Ohio is an EEO employer - M/F/Vets/Disabled The LPN oversees the clinical care of the elders that he/sheis assigned.

    The nurse works with theClinical Support Team (CST) to accomplish the mission of MHCO in caring forelders.


    QUALIFICATIONS/SKILLS REQUIRED:
    Current LPN license in the State of Ohio Ability to recognize the autonomy and dignity of all elders Communicate a sense of caring, concern, sensitivity, and dignity for elders Place decision making in the hands of the elders whenever possible and appropriate Make prompt and accurate judgments regarding elder care and emergencies Able to be flexible, adaptable, and creative Customer service oriented Dependable and reliableKEY RESPONSIBILITES/ESSENTIAL JOB FUNCTIONS:

    • Provides care in accordance with state and federal guidelines, Nursing Standards of Practice, and Maple Crest Nursing Policies and Procedures.
    • Obtain an oral update on each elder from the Elder Assistant during each shift.
    • Receive and provide a daily nursing report of elders.
    • Visit each elder during each shift to monitor clinical status. Determine if the clinical care needs of the elder are being met. Complete clinical assessments. Consider the primary diagnosis and risks specific to the individual elder.
    • Administer and document treatments and medications.
    • Work cooperatively with the Elder Assistant, ALC, and the Clinical Support Team.
    • Support the Elder Assistants in creating a positive rhythm in the AL area.
    • Participate in care planning which considers physical, emotional, and spiritual needs of each elder. Monitor adequacy of the care plan. Review compliance with and update the care plan including the input from the support staff.
    • Observe/assess for and report changes in elders' condition to ALC, attending physician, family or designated contact person.
    • Identify and provide any clinical teaching needs of the Elder Assistant/elder/family.
    • Act as a role model in the implementation of MHCO/Maple Crest policies and OSHA Standard Precautions. Report observations/findings of non-compliance to the ALC or Executive Director.
    • Maintain emergency supplies and operate equipment for elder care

    Example:
    oxygen equipment. Possess ability to perform Heimlich maneuver and CPR while maintaining a current certificate.

    • A portion of nurse time may be spent providing elders with personal care services.
    • Actively attempt to replace a nurse who calls in ill, etc.
    • Participate in the on-call schedule on a routine basis as determined by the ALC.
    • Participate in development of policies and procedures to improve nursing care programs.
