    What you will do

    Provides virtual clinical care to patients within the recognized scope of practice for the profession and professional licensure requirements
    Provides clinical triage and clinical support using established protocols during in-bound and out-bound telephone communication with patients, family, caregivers and other members of the healthcare team
    Assists in coordination of incoming patient transfer requests with receiving facility and physicians
    Demonstrates technical skills in care delivery that result in safe practice and positive care outcomes
    Possess and obtain working knowledge of external and inter-facility protocols and standards of practice
    Demonstrates clear communication and effective collaboration to optimize patient care and clinical operations
    Delivering exceptional customer service while facilitating communication across multiple disciplines
    Work with all Center for Patient Flow Management (CPFM) staff in a team orientated, collaborative approach
    Responsible for promoting a Best Place to Work Culture and positive and professional work environment
    Actively participates in the development and implementation of performance/quality improvement
    QualificationsEducation Qualifications

    (BSN) Bachelor's Degree in Nursing Required

    Experience Qualifications

    3+ years of acute care hospital in-patient nursing Required and
    3+ years clinical experience in home health or similar setting Preferred and
    Experience with care management plans. Required and
    Experience with Medicare, Medicaid and commercial payer regulations Preferred

    Skills and Abilities

    Telephone triage (Preferred proficiency)
    Excellent interpersonal and communication skills both written and oral. (Required proficiency)
    Strong organizational and multi-tasking skills. (Required proficiency)
    Demonstrates a professional demeanor when interacting with patients and customers. (Required proficiency)
    Strong computer skills including all MS platforms. (Required proficiency)
    Excellent ability to perform triage in order to determine appropriate facility, bed type, service, etc. (Preferred proficiency)

    Licenses and Certifications

    Registered Nurse (RN) in the State of Ohio Upon Hire Required
