    We have a fantastic opportunity for Full-Time Case Manager with a CASAC or CASAC-T License at Montefiore Mount Vernon Hospital.


    JOB SUMMARY
    This is an entry level professional position without supervisory duties.

    Under the supervision of the Program Manager, the case manager focuses on improving the quality of life of people living with Pervasive Behavioral Health, by ensuring receipt of services such as; entitlements, benefits, housing, transportation, legal, community/social programs.

    The Case Manager will assist patient to overcome barriers to Health & Wellness. The Case Managers responsibilities involve both on site and community service delivery.

    The Case Manager will be expected to provide a variety of services such as;Conduct intake and comprehensive assessments of needsTreatment planning and implementation of treatment planService coordination, monitoring and follow-upCoordinating of referrals to community/service providers (DSS, Housing, DCMH, Legal, Financial)Provide advocacy and assistance navigating the social services system to obtain entitlementsPromote and support self-help & independent livingFacilitate and/or co-facilitate support groupsParticipation in multi-disciplinary team meetings and supervisionMaintain up-to-date records as required by the program procedures and regulatory agency requirements.

    Attend & complete ACT Core Trainings as required by NYSOMH

    POSITION QUALIFICATION:


    An associates degree in Health and Human Services, Social Work, Psychology, Liberal Arts required and one year minimum work experience with individuals diagnosed with mental health issues.

    Bachelors Degree in above disciplines can substitute for one year of experience.
    Individual must possess good verbal, writing and computer skills.
    Bilingual (Spanish/English) preferred.
    Valid NYS Driver's licenseCASAC license required
