The Care Coordinator uses a motivational, stage-matched approach to coordinate care for clients throughout all phases of care to collect, analyze, and communicate information on the status of client health and current needs.This is a full-time position working Monday-Friday,
8:00AM - 5:00PM.
Qualifications
- Bachelor's degree in Counseling, Psychology, Social Work or other human services field.
- High School diploma with at least 4 years of experience or combination of higher education.
