    The Children's Home of Reading Youth and Family Services is seeking a Full Time Foster Care Case Manager, with great organization and time management skills, servicing the Berks and surrounding counties. Position provides SWAN adoption services to children and families preparing them for permanency. This is NOT a remote position. You will be required to work hours in the office, field, and home. This position will require transporting clients in your vehicle.

    PURPOSE:
    The Case Manager is responsible for safety and services to children and families in foster care. Tasks include facilitating team meetings, development of individualized service plans, coordination of individualized services across all settings, including the home, school, and community environment, and documentation of support related activities and services.

    Requirements:
    • Bachelor's degree with a focus on Human Services ( social work, psychology) REQUIRED
    • One (1) year verified experience in case management or counseling with children and families with one (1) year experience. (Drug and alcohol programs - one year drug and alcohol experience required.)
    • Must be at least 21 years of age.
    • Valid PA driver's license (or ability to obtain such license), and a good driving record, with proof of auto insurance coverage.
    • Reliable transportation of vehicle with at least $100K/$300K insurance coverage.
    • PA Act 33/34 clearances and DPW FBI clearance.
    • Pass all pre-employment physical and drug screening
    • Must meet all requirements of Title 8, United States Code, Section 1324-A of the United States Department of Justice/immigration and Naturalization services Employment Eligibility Verification.
    • Availability and willingness to work a flexible schedule.
