    Synchrony Rehab, a subsidiary of Trilogy Health Services LLC, is currently looking for a licensed

    Occupational Therapist

    at Homewood Health Campus, an innovative Skilled Nursing and Assisted Living health campus in Lebanon, IN. A valid Occupational Therapy (OT) state licensure is required for this position.

    Job Description Summary:


    Responsible for delivering a comprehensive range of occupational therapy services, including assessment, treatment planning, and therapeutic interventions within an interdisciplinary setting that aligns with professional standards and ethical practices.

    Demonstrates responsibility and contributes to program enhancement, quality advancement, problem-solving, and productivity improvement within a collaborative interdisciplinary setting.


    ROLE AND RESPONSIBILITIES

    • Provides therapy services such as assessment, treatment planning, and therapeutic interventions in various care settings.
    • Performs initial assessments.
    • Contributes to interdisciplinary care plans.
    • Administers appropriate therapeutic interventions.
    • Assesses patient response to treatment and offers feedback to the team.
    • Documents patient care progress and ongoing treatment needs.
    • Participates in survey preparation activities.
    • Educates patients and families on tasks and resources for continued functional independence.
    • Serves as a resource and provides training in occupational therapy knowledge and skills to employees.
    • Supports student program development as needed.
    • Contributes to cost-effective care delivery.
    • Reports resident complaints/grievances to appropriate personnel.
    • Reports incidents of resident abuse immediately to the relevant authorities.
    • Travels to different locations as required.
    • Adheres to resident rights policies.
    • Maintains professional competencies.

    QUALIFICATIONS AND EDUCATION REQUIREMENTS

    • Must have completed a bachelor's degree program recognized by the CAHEA of the AMA in association with the AOTA (or an equivalent foreign program), and possess a valid license to practice.
    • Experience with adult and geriatric populations preferred, particularly in a skilled nursing setting; experience with a disabled population is beneficial.
    • Must hold an unrestricted state license.
    • Must have current CPR Certification.
    • Two (2) years' experience in healthcare, senior living, or long-term care preferred.
    • Proficient in Microsoft Suite.

    PHYSICAL REQUIREMENTS
    Sitting, standing, bending, reaching, stretching, walking, and lifting up to fifty (50) lbs. occasionally. Requires coordination of arms, legs, and the whole body.

    Must have adequate vision and hearing or use aids to ensure full job requirements are met.

    Ability to see details up close and operate office equipment.
    Good verbal and written communication skills.
