- Conduct initial and ongoing assessments, including OASIS assessments.
- Develop and revise treatment plans with physician approval as necessary.
- Participate in the development and implementation of interdisciplinary client care plans.
- Instruct and counsel clients and families on physical therapy programs.
- Supervise Physical Therapist Assistants in accordance with regulations.
- Communicate with physicians and team members regarding client status and care.
- Document clinical/progress notes promptly and accurately.
- Graduate from an accredited Physical Therapy program.
- Valid Florida PT license.
- Current BLS Certification.
- Valid driver's license with reliable and insured vehicle.
- Preferably one year of experience as a Physical Therapist, with home healthcare experience beneficial.
- Proficiency in electronic medical records preferred.
- Ability to work independently with excellent observation, judgment, and communication skills.
