Nurse Practitioner - Advanced Practice Provider

Location:
Fairview (Camden, NJ)

Salary:
$105,000-148,500**Wider range available depending on experience and location

Schedule:
Full-Time, Monday through Friday, 8:00 AM - 5:00 PM

Role Description:

The role of a Nurse Practitioner at Oak Street Health is to deliver high-quality primary healthcare to Medicare adults, ensuring their well-being and reducing hospital visits.

Nurse Practitioners work independently and in partnership with physicians as needed.

Nurse Practitioners establish enduring patient relationships, fostering trust and enhancing patient engagement with their treatment plans.

Nurse Practitioners operate within our outpatient clinics as part of a collaborative healthcare team, which includes physicians, nurse practitioners or physician assistants, medical assistants, a dedicated medical scribe, and support from registered nurses and care managers.

We collaborate with a network of top-tier specialists and hospitals for specialized and acute care services.

Thus, our healthcare providers concentrate on in-clinic care:
primary care, care coordination, and public health initiatives.

Core Responsibilities:
Providing exceptional primary care services.

Conducting office visits for routine and acute medical needs.

Performing Annual wellness visits and health risk assessments, emphasizing comprehensive health evaluation and accurate documentation.

Coordinating care with other healthcare providers, specialists, testing facilities, and organizations.

Leading public health programs, alongside the Care Team (e.g., ensuring all eligible women receive evidence-based breast cancer screenings every 2 years)

Supporting the care team with telephone triage and outreach.

Educating patients on their health conditions, treatment plans, and therapies.

Engaging in Oak Street Health promotional initiatives.

Conducting home visits when necessary.

Performing other assigned duties.

This position reports to the Center Medical Director and collaborates closely with operational leadership.

Required Qualifications

Master's Degree in Nursing, Doctorate of Nursing Practice, or completion of an NP Certificate Program

National certification in at least one of the following areas:

Family Nurse Practitioner

Adult-Gerontology Primary Care Nurse Practitioner

Adult Nurse Practitioner

Gerontological Nurse Practitioner

Active state Nurse Practitioner license without probation

Active DEA license

Authorization to work in the United States

Demonstrates 'Oaky' values

Preferred Qualifications

Experience in: internal medicine, geriatrics, or family medicine

Proficiency in HCC (Hierarchical Condition Categories) documentation, ICD-10 Coding, and Health Risk Assessments (HRAs).

Passion for collaboration and interdisciplinary teamwork

Keen on contributing to an innovative model focused on data-driven public health initiatives

Fluency in multiple languages
