    Job Duties:

    • Serve as main point of contact for transitions of care between care coordinator partners
    • Provide direct linkage to outpatient care for high-risk and recently discharged behavior health clients
    • Participate in treatment planning, safety planning, caring contacts and follow-up services, as needed
    • Update client electronic health records, as needed
    • Stay up-to-date on training, resources, and best practices for safe transitions of care
    • Document all interactions according to agency standards
    • Participates in collaborative activities of the care coordination partners

    Skills and Proficiency:

    • Demonstrate core values through work that is person-centered, relationship-focused, and trauma-informed
    • Engage clients in a collaborative and caring relationship, using interpersonal skills, knowledge about suicide prevention, and attitude consistent with Zero Suicide
    • Promote health integration and wellness activities, assist with health screenings and linking to coordinated health care
    • Time management, and task oriented documentation
    • Collaboration and teamwork, consulting and facilitation, coaching and mentoring
    • Computer systems and MS Office, including Word, Outlook

    • Minimum Qualifications: Education: Bachelor's degree in Public Health, Psychology, Social Work, or related field of study.Licensure: None required Experience: Experience with care coordination, case management, and/or patient discharge processes. Familiarity with electronic health records (preferred). Knowledge of safety planning practices and other elements of the Zero Suicide framework (preferred). Must be at least 21 years of age. Negative Drug Screening prior to hire and throughout employment.Clear Criminal Background Check throughout employment with Suncoast Center and local checks prior to hire and annually along with DCF Level II prior to hire and as required by DCF, and Must have valid Florida Driver's License Motor Vehicle checks prior to hire and annually with acceptable driving record maintained throughout employment along with reliable vehicle, valid registration, and valid vehicle insurance. (must be in compliance with Florida Laws and Administrative Codes with fingerprinting and determined to be of good moral character).Working Conditions: Primarily in the office. Must be able to efficiently and effectively handle telephone calls and computer documentation. Schedule may be irregular if emergencies arise.
