- Conducts initial and/or comprehensive nursing assessments, identifies primary care goals, formulates the care plan following state-specific guidelines, and submits precise, thorough, and timely documentation in conjunction with physician directives.
- Performs comprehensive patient evaluations at specified intervals and develops appropriate care plans.
- Verifies that patients meet the home health eligibility and medical necessity criteria outlined by the payer source.
- Active RN licensure in the state of practice
- Current CPR certification
- Valid Driver's License, auto insurance, and access to a reliable vehicle or public transportation
- Minimum of one year of clinical experience as a Registered Nurse. Recent LPN clinical experience may substitute one year of Registered Nurse experience. RN license must be unrestricted.
