Grounded by our belief that outstanding care is best delivered in a team-based environment, our Hospice Admissions Registered Nurse will partner with our field staff and leaders to provide the best support to the patients that we serve.Responsibilities
- Hospice patient identification, follow-up, and management in the hospital
- Manage Hospice referrals for assigned area, which may come from several sources including but not limited to: Hospitals, Discharge Planners, Physicians
