Job Summary
We take patients out of emergency rooms and turn them into members of our healthcare system, providing comprehensive care. We engage members in their communities to find those who need us most.Responsibilities
- Focusing on complex clinical care for patients with multiple chronic conditions.
- Developing transitional care plans and tracking patient progress along the continuum of care.
- Treating acute exacerbations of chronic conditions within our center or referring patients to higher levels of care when necessary.
