    The transition coordinator identifies, coordinates or provides continuity of care and appropriate levels of care under the direct supervision of an RN for PathWays for Aging members. The transition coordinator is responsible for coordinating care management and utilization management information as members move into and out of the health plan.

    Primary Responsibilities:

    • Serve as a liaison for post-transition concerns
    • Aid members who lose nursing facility level of care or are ready for discharge from a nursing facility to ensure a successful discharge or transfer to another residential setting
    • Work with the Member Advocate Coordinator and other member-focused departments
    • Outreach to and engage a member transitioning between programs to confirm continuity of care during and after the transition
    • Ensure continuity and coordination of care and member and provider communication through the initial transition, ongoing benefit plan, and MCE transfers
    • Complete or arrange for necessary assessments to identify the member's physical health, behavioral health, community and social support needs
    • Collaborate with the member's care team to develop a transition of care plan
    • Responsible for efficiently providing the receiving entity with information on any current service authorizations, utilization data and other applicable clinical information such as prevention and wellness programs(s), case management or care management notes when a member transitions to another source of coverage
    • Facilitate the honoring of existing authorization of services during the transition to confirm continuity of care

    Required Qualifications:

    • LPN or LVN with current unrestricted licensure in Indiana
    • 1+ years of related professional experience with a similar population as those in the PathWays for Aging program or older adults
    • Related professional experience as a care manager, service coordinator, transition coordinator, health specialist, social services coordinator or licensed behavioral health professional in the state of Indiana
    • Reside in Indiana
