As a Client Relations Executive at Pathways Home Health & Hospice, you will be responsible for making essential contributions to the achievement of the organization's objectives as a field-based business development representative.Pathways provides care at home or in settings such as assisted living, a nursing home, or the hospital. We have offices in Sunnyvale and Oakland. Patients and their families know us for our personalized high-quality care delivered with empathy kindness and respect.
Supports Pathways' mission to exceed expectations of our customers associates shareholders in delivery of health care support services in way that caring family provides,
Adheres corporate compliance program including confidentiality HIPAA health protected information must meet established monthly admission target(s) provided by supervisor determined branch monthly budget identifies qualifies healthcare relationships within regional territory focuses skilled nursing facilities assisted living communities community based residential facilities physicians other healthcare providers hospital systems source referrals admissions makes site visits telephone calls month expand group referral sources sources admissions develops maintains relationships customer through regular contact follow-up procedures acts key contact client troubleshooting conflict resolution responsible developing yearly business plan region includes strategies exceeding established budgets goals develops maintains accurate data RSL customer each key referral source provide client needed build strong ties deliver satisfaction communicates daily basis give receive vital client information completes reports detailing field activity results weekly basis represents pathways relevant professional organizations community assists proposal preparation presentation analyzes recommendations contracts assists educating community about services all duties assigned follows referral patient pathways assist facility discharge coordinators coordinating quality home care clients following manner determines patient eligibility secures patient admissions effectively communicate appropriate disciplines involved potential clients provides value added counsel teaching problem solving.
