Job summary
This position is a Caseworker I with primary duties as Chaffee & Foster Youth in Transition Caseworker. Provides intake & needs assessments, makes referrals to community resources & provides case mgmt for clients in at-risk populations.The caseworker will also be responsible for coordinating services for youth to include assistance with securing housing, enrolling in Medicaid, obtaining vital documents & records, accessing education & employment resources,
