- Assume initial responsibility for patient.
- Review discharge order, referral and instructions from treating physician or medical center.
- Independently perform initial, head to toe, physical assessment on patient, utilizing independent discretion and advanced knowledge based on patient needs and examination of patient, to determine the following:
- Review and reconcile all patient medications.
- Complete all necessary initial patient documentation within forty-eight (48) hours of admission to home health (OASIS Documentation, Patient Consents, etc.).
- During SOC visit, the nursing specialist must:
- Assist patient with coordination of initial, seven (7) day follow up with treating physician. Follow up with patient to determine if seven (7) day follow up appointment was completed.
- Communicate with all necessary disciplines (RN, OT, PT, ST, HHA, MSW) for proper continuation of care throughout patient episode.
- Perform other duties as needed pursuant to independent review of patient.

o Determination of all disciplines (RN, OT, PT, ST, HHA, MSW) needed by patient; and,
o Initial ordering of any necessary wound care and/or other supplies for patient needs.
o Communicate with treating physician, in the presence of patient, to discuss and verify all medications, orders, disciplines and to identify all additional physician consultations needed to care for patient and to be included within the plan of care.
