    Primary Care Outpatient

    Welcome to this exciting opportunity at Yale New Haven Health!

    About the Position:

    • We are seeking a highly skilled Advanced Practice Provider (APP) to join our Primary Care team in New London, CT.

    This is a full-time position with 40 scheduled hours per week, Monday through Friday from 8am-5pm. We encourage a four-day work week for those who qualify.



    Job Responsibilities: As an APP in our Primary Care team, you will be responsible for providing comprehensive health assessments, developing and implementing individualized treatment plans, prescribing medications and ordering therapies as needed. You will also be expected to provide patient education and counseling on health maintenance and disease management.

    Licensure/Certifications: The ideal candidate must have active PA or NP licensure in family medicine specialty certification with current ACLS/BLS/PALS Certification DEA License
