- Submit necessary documentation after patient visits.
- Participate in staff meetings, educational programs, and community events.
- Communicate needs and solutions to supervisors.
- Adhere to nursing standards and stay updated on clinical practices.
- Engage in quality improvement projects and clinical reviews.
- Effectively communicate with healthcare team members and physicians.
- Provide nursing services and administer treatments as directed.
- Attend IDT meetings and offer patient education.
- Perform other assigned duties.
- High school diploma or equivalent.
- Current nursing license and a minimum of two years of experience as a Licensed Practical/Vocational Nurse.
- Experience in skilled nursing, preferably in hospice, home health care, oncology, or emergency care.
- Mathematical proficiency including basic calculations.
- Strong written and verbal communication skills.
- Ability to problem-solve and handle various situations.
- Computer proficiency, including Microsoft Office and EMR.
- Understanding and acceptance of hospice philosophy.
- Valid driver's license and auto insurance.
- Ability to work with terminally ill patients and their families.
- Fluency in English.
