Position Description Anoka County Community Social Services and Behavioral Health (CSS/BH) currently has a position available as a Social Worker in our Long-Term Services and Supports (LTSS) service area.

Qualified candidates may be a Social Worker, Registered Nurse, or have a degree in a related field. LTSSprovides intake and referral services to individuals seeking assessments related to long-term services and support.

This position conducts annual, in-person comprehensive assessments utilizing the MnCHOICES assessment tool todetermine eligibility and the need for home and community-based services.

Pay & Benefits Salary:
$70,012 to $84,011 annually ($33.66 to $40.39 per hour Anoka County Salary Schedule Grade 34: $70,012 to $98,009 annually ($33.66 to $47.12 per hour). 24 days of paid flexible time off and up to 12.5 paid holidays. Work Location This position will work at the Anoka County Government Center, located at 2100 3rd Ave, Anoka, MN This position may be eligible for flexible work arrangements, including hybrid work, with some days working remotely and some days working in the office. m. - 4:30 p.m. Job Duties and Responsibilities These examples are not all inclusive and are intended to be illustrative of primary responsibilities of an Anoka County Social Worker. Identify people and communities in need of help. Evaluate clients (existing, new and prospective) based on their needs, limitations and desires. Assess clients' needs, situations, strengths, and support networks to determine their goals. Build rapport with clients and their families. Assist clients in receiving services by telephone and in person. Respond to crisis situations such as child abuse and mental health emergencies. Help clients adjust to changes and challenges in their lives. Monitor clients' situations, and follow up to ensure that they have improved. Advocate or raise awareness with and on behalf of their clients. Maintain case files and records. Provide leadership, coaching, and/or mentoring to a subordinate group as assigned.

Qualifications and Requirements:

Minimum Knowledge, Skills, and Abilities Needed Bachelor's degree in social work, nursing, or a closely related field, OR current licensure as a registered nurse.

Two years (2,080 annual hours) of job-related experience.

In lieu of the required education, an equivalent combination of education and experience may be substituted on a year-for-year basis.

Valid driver's license and vehic e available for business use. Preferred Knowledge, Skills, and Abilities Needed Master's degree or higher in social work, nursing, or a closely related field. Three years or more (2,080 annual hours) of job-related experience. Experience in Disability, Elderly/Alternative Care waiver case management. Knowledge or experience in working with clients with developmental disabilities or related conditions. Experience in Rule 185 case management. MnCHOICES Certified Assessor. Fluent in a language in addition to English. Experience with state data systems such as Minnesota Medicaid Management Information System (MMIS) and Social Services Information System (SSIS). Physical Demands and Work Conditions Standard office environment. Occasional lifting of pounds. Occasional travel to other county work sites as needed. Travel between work sites may require driving a county vehicle and/or a personal vehicle.
