- Collaborate with clients and the treatment team to develop, implement, and revise treatment goals and plans. Make immediate adjustments as needed based on clients' changing needs
- Participate in comprehensive assessments of clients' psychiatric history, mental status, physical health, drug/alcohol use, education, social development, and more
- Provide service coordination for a group of clients, educate their families, and advocate for clients' rights
- Offer tailored support in symptom management, substance abuse services, time structuring, education, employment, and daily living activities
- Provide practical assistance, advocacy, problem-solving, and training to help clients meet their daily living needs
- Attend team meetings, treatment planning sessions, and weekly supervision
- Complete all necessary documentation as per agency guidelines
- Provide crisis intervention during evenings, nights, and weekends
- Collaborate with community agencies and families to ensure a coordinated treatment approach
- Assist clients with transportation as required; driving is a key aspect of this role
- Other duties as assigned
- High School diploma or equivalent (GED) required
- Preferably holds an Associate's or Bachelor's degree in a human service-related field
EXPERIENCE/TRAINING/REQUIREMENTS - Minimum four (4) years of relevant experience or a suitable combination of education and experience for candidates with a High School diploma or equivalent
- Minimum two (2) years of relevant experience or a suitable combination of education and experience for candidates with an Associate's degree
- One (1) year of experience is required for candidates with a non-human service Bachelor's degree
- All experience should involve working with individuals with psychiatric, substance use, and/or developmental disabilities
- Must possess a valid driver's license and meet agency automobile coverage standards
