- Provides delivery of direct and indirect patient care
- Responsible for providing care to medically complex patients
- Documents all activities related to the patient's care according to thepatient's plan of care
- Maintains confidentiality at all times with patients, medical staff andco-workers
- Provides a safe, comfortable and therapeutic patient environment
- Experience in acute rehabilitation for vents, wound care, cardiac,stable post trauma patients
- Adheres to and supports the policies/procedures/goals/objectives of thehospital in an attempt to provide quality patient care Revises plan of care according toevaluation, changes in medical plan of care and nursing interventions
- Performs interventions according to identified priorities, plan ofcare, and the hospital policies and patient care outcome standard
- Identifies and prioritizes actions based upon patient care requirements
- Supports professional practice by observing the following: Usesknowledge and practices in accordance with the Nurse Practice Act; Joint Commission, state andlocal laws; and Patient's Bill of Rights hospital policies.
- Performs other duties as assigned/necessary
- Current nursing license for the state in which the nurse practices in
- One (1) year nursing experience in a long term acute care facilitypreferred
- Ability to effectively elicit/provide information to and fromappropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strongcommunication skills; proficiency in the English language may be required
- Computer proficiency required
- Must be at least 18 years of age
