Job DescriptionSaint Luke's Primary Care Clinics is hiring for a Regional Nurse Care Coordinator for their Population Health team. This Registered Nurse will provide care to a diverse patient population.This role works collaboratively with multi-disciplinary teams to coordinate care and provide patient education to patients and their loved ones.
This Nurse position will be responsible for treating the whole patient and assist our patients in becoming more independent and take ownership of their health and disease management.
This position will assist with clinical performance improvement activities and partner with external and internal partners to ensure we provide care to the total patient.
This is a strategic RN position that will require you to build strong relationships with patients, care teams, and other internal and external partners predominately over the phone.

Engage with the multiple regional Saint Luke's Physician Group clinics to impact Patient Care within specific payment structures.
Drive engagement strategies for improving physician and staff patient care practices based on clinical guidelines, best practices internal and external benchmarks, and claims-based data.
Supports and ensures the overall success of the patient's healthcare by collaboratively working with patients physicians practice teams and health plans to select guide and direct patient care while integrating key features of the medical home.
