- Assess clients personal, medical, emotional, social, and environmental situation through home, community, and office visits.
- Act as a liaison for clients, Trilogy, and housing provider to facilitate long term housing stability and safety by completing referrals to additional services as indicated.
- Coordinate the establishment of an individual recovery plan with the client, clients support system, and other care providers.
- Assist clients in identifying signs and symptoms of de-compensation. Work collaboratively with clients to identify coping skills to assist in managing symptoms.
- Assist clients in learning and improving independent living skills, i.e., personal hygiene, housekeeping skills, cooking/nutrition, and shopping for food and personal items.
- Educate and assist clients with applying for entitlements, i.e., Supplemental Security Income, Social Security Disability Insurance, Medicare, Medicaid, Ride Free and LINK.
- Provide psychoeducation, medication training and monitoring to clients according to Trilogy policy.?
- Coordinate with team nurse/pharmacy to ensure client medication accuracy.?
- Documenting in real time on medication administration record (MAR)?
- Conduct assessments with clients who are determined eligible for SOAR and initiate paperwork with clients by filing initial documentation of representation with SSA office.?
- Complete interviews with clients to gather information to complete SSI/SSDI application.
- Conceptualize a clients case to advocate for disability benefits and remain organized and timely during the filing process to follow up on appeals/reconsiderations as needed.
- Coordinate case management services with partners and help with providing case management services to participants when needed?to:
- Gather medical records and other information to complete SSI/SSDI applications using the SOAR method.
- Write SOAR Medical Summary Reports for consumer applications.
- Accompany clients to SSA, PCP, Psych, and other specialists to provide support related to benefits, housing, substance use, budgeting, social support, and medication.
- Communicate effectively within the team model for a multi-disciplinary approach to client care.
- Complete documentation and progress notes for services provided.?
- Perform other related duties and/or projects as assigned.
- Must have one of the following requirements:
- Bachelor's degree in counseling and guidance, rehabilitation counseling, social work, education, vocational counseling, psychology, pastoral counseling, family therapy, or related human service field
- Bachelor's degree in any other field with two years of supervised clinical experience in a mental health setting
- Licensed Practical Nurse under the Nurse Practice Act [225 ILCS 65]
- Certificate of Psychiatric Rehabilitation from a DHS-approved program, a high school diploma/GED, and two years experience of experience in a mental health setting
- CRSS certification through IAODAPCA
- Family Partnership Professional certificate from and in good standing with IAODAPCA
- Occupational Therapy Assistant licensed under the Illinois Occupational Therapy Practice Act [225 ILCS 75] with one-year experience in a mental health setting.
- High school diploma or GED and five years of supervised clinical experience in a mental health setting
- Valid Illinois drivers license with daily access to a well-maintained vehicle with $100,000/$300,000 liability insurance??
- Experience working with people with mental health and/or substance abuse diagnoses preferred.
- Experience in community-based services preferred.

The Residential SOAR Specialist (RSS) will provide support services to clients residing at Ogden Park Apartments and other Trilogy clients. They will assess the clients personal, medical, emotional, social, and environmental situation to plan for treatment course through home, community, and office visits. They will provide individually based motivational treatment and support to clients to assist them in their recovery from mental illness. They will be an advocate for clients and link them to community services as needed.
The RSS will be SOAR trained and assist in navigating the system of gathering relevant data and working with clients to obtain benefits while building a therapeutic relationship and remaining on top of deadlines.???They will provide assertive outreach, obtain clients medical records, manage communication with the Social Security Administration (SSA) and Determination Services (DDS), provide a detailed narrative, and obtain a co-signature from medical providers.? They will provide client-centered, strengths-based, and trauma-informed services to clients with severe mental illnesses and co-occurring substance abuse/addiction disorders utilizing a harm reduction approach.???

The RSS must complete online training for SOAR and successfully receive their SOAR certification within 90 days of hire and will provide at least 60-75% of all community support services in the community. They will complete case management tasks with clients within a shared caseload. The RSS will also provide crisis/on-call coverage as directed.
