Job description
36 Hours Weekly; Friday, Saturday and Sunday.
Qualification Requirements
- Graduate of an accredited school of registered nursing; BSN preferred.
- Currently licensed as an RN through the Ohio Board of Nursing; must be able to obtain a nursing license in Michigan.
- Minimum 2 years of medical-surgical nursing experience required; previous experience in homecare, nursing home, and/or hospice experience is required.
- Minimum 2 years experience in a supervisory/leadership role preferred.
- Has working experience charting in an EMR system, NetSmart system preferred.
- Possesses strong verbal and written communication skills; demonstrates ability to communicate effectively with patients, families, and team members.
- Possesses clinical expertise related to critical thinking, problem-solving, and technical skills.
- Demonstrates strong organizational, time management, and prioritization skills.
- Demonstrates strong analytical skills.
- Has the ability to work independently and within an interdisciplinary team.
- Must be a licensed driver with an automobile in good working condition and is insured in accordance with state and agency requirements.
- Understands the hospice philosophy, principles of death/dying, concepts of pain/symptom management, and adult teaching/learning principles.
Essential Job Responsibilities
- Provides direct supervision of assigned staff and holds employees accountable to established agency performance standards. Inspects the work of team members to make sure that quality and performance standards are upheld. This includes but is not limited to: going on clinical visits, reviewing productivity reports and clinical documentation.
- Leads by example and upholds the culture, standards, and values of the agency.
- Provides to team members assistance with clinical problem solving to ensure the needs of patients and families are met.
- Provides timely performance feedback to employees and conducts annual performance evaluations on time.
- Teaches and mentors team members and ensures continued improvement of employees' knowledge, skills and abilities.
- Knows the NPR model and manages expenses. Identifies and initiates opportunities to improve quality and increase efficiencies.
- Assists with the development and implementation of change processes to ensure employees are able to function at the highest level to be efficient and productive.
- As a leader in the organization, has responsibility to help the agency achieve its short-term and long-term strategic goals.
- Conducts monthly staff meetings and ensures the effective and positive communication of agency information and goals.
- Organizes staffing resources to meet patient needs and ensures caseloads are equitably assigned; ensures staffing is within budget.
- Participates in the interview and selection process of new team members, as well as the orientation and training of new team members.
- Maintains open communication with Human Resources regarding employee performance issues; ensures documentation of employee performance coaching and counseling.
- Performs audits of employee documentation to ensure the employee is performing within his/her scope of practice, is in compliance related to his/her license, and is performing established best practice. Completes ongoing audits of every discipline monitoring regulatory compliance and HNWO standards.
- Maintains knowledge and understanding of Medicare, Medicaid, Joint Commission, state and agency standards/regulations, and ensures the team's compliance with these standards/regulations.
- Oversees insurance authorizations.
- Provides oversight of the recertification process and assures documentation is timely, accurate and complete.
- Organizes and attends weekly IDT.
- Responsible for ensuring self and staff learn and utilize technology made available by the agency to increase efficiency, communication, and performance of duties.
- Participates in the agency's Quality Improvement program and ensures the successful implementation of performance improvement initiatives.
- Attends required meetings and completes all mandatory training.
- Holds in strictest confidence all patient information and discloses information only to persons authorized by Hospice.
- Performs other duties as assigned.
