  • Job Summary:  Assists in activities of care coordination and care management for Salem Clinic patients. 

    Required Knowledge, Skills, Abilities: 

  • Possession of an Oregon State Licensed Practical Nurse license.
  • Ability to meet Oregon standards and scope of practice for LPN work in a variety of nursing roles.
  • Two years' experience working in a medical facility.
  • Knowledge of principles and skills needed to provide nursing assessment, patient education, health promotion/wellness, and the development of a focused plan of care.
  • Knowledge of common disease processes and preventive care guidelines.    
  • Knowledge of medications and their effects on patients.  
  • Knowledge of patient education principles the ability to assist patients in caring for themselves.
  • Strong written, verbal, and telephone communication skills.
  • Excellent organizational skills with the ability to pay close attention to detail.
  • Ability to work in a high volume, fast paced environment.
  • Ability to exercise initiative and independent, sound judgment.
  • Ability to establish effective working relationships with staff and physicians.
  • Ability to use a computer.   Proficient keyboarding skills.
  • Essential Functions:

  • Skill in applying and modifying the principles, methods, and techniques of LPN nursing practice to provide focused patient care planning and coordination. 
  • Independently develop a focused plan of care prioritizing interventions from patient care plans during transitions of care, for patients identified as high risk, or patients with high ED utilization. 
  • Utilize reports in Epic and from outside agencies to track patients and discharges from ED, hospital and skilled nursing facilities.   
  • Utilize nursing judgment to determine content and cadence of outreach to patients. 
  • Review patient record to identify labs/imaging/mental health/ specialty notes that are pertinent to patient's current diagnoses/needs and enter standing orders and or communicate needs to the provider.
  • Develop strategies and interventions to improve patient's compliance, understanding with plan of care, and health outcomes. 
  • Provide medication and allergy reconciliation and related education for understanding of patient's medical needs and medications. 
  • Request appointments within appropriate time frame for patient's needs or as indicated by provider.
  • Implement focused plan of care interventions.
  • Work in collaboration with Provider and other pertinent care team members regarding patient care plan.  Evaluate patient progress and response to focused plan of care interventions.  Communicate status and issues or concerns with appropriate parties.
  • Ensure that there is communication with patients, family, and outside members of the care team as appropriate to maximize health outcomes.
