    Description

    JOB SUMMARY:
    Transportation CNA shall work under the direction of and be responsible to the Director of Nursing Services to provide transportation services for facility Residents and other duties as required by the DON or Administrator

    EDUCATION:
    A high school diploma is required. Must be able to communicate well, verbally and in writing.


    QUALIFICATIONS:
    Must be a certified nurse's aide.
    Have a clean driving record with a valid driver's license in the state working in.
    Able to communicate effectively with co-workers, residents, families and physiciansSociable, courteous, and sympathetic.
    Have Considerable initiative and judgment in promoting favorable public relationsAvailable to be on call.
    Must have reliable communication service (home phone/cell phone).


    WORK RESPONSIBILITIES:
    Work with nursing staff to schedule doctors' and other medically related appointments and handle arrangements for those appointments.
    Drive the facility van to transport residents to and from their medical appointments.

    Work with nurses to make sure that proper medical paper work is provided and in order for all residents going out on medical visits/appointments.

    Maintain confidentiality regarding all resident inquiries. Refer all complaints and/or incidents to the DON or Administrator.

    Maintain safety and comfort of residents as much as possible during transportationKeep a detailed log of time spent on transportation runs.

    Maintain the routine van mileage log when driving.
    Responsible for routine van maintenance and upkeep including scheduling of routine service checkups.

    Assist in feeding of residents at mealtimes when in the facility and providing resident care as directed by the DON.Any other duties assigned by the director of Nursing Services.


    PHYSICAL DEMANDS:
    Standing, sitting, bending, stooping. Able to get in and out of van and able to assist residents in and out of van. Must work throughout all nursing areas of the facility and transportation areas.

    May require working irregular hours and may be exposed to communicable diseases and/or body fluids, medical preparations, toxic substances and occasionally ionizing radiation.
