    POSITION SCOPE AND LOCATION

    The NP practices under the standard procedure established for the NP role consistent with client policies and in accordance with requirements set forth by the Ohio Board of Nursing for Advanced Practice Registered Nursing (Chapter

    WHAT YOU'LL DO

    • Provides medical care for patients with complex, chronic and/or acute conditions
    • Participates in acute and chronic care management of patients, specializing in assessment and documentation of high-risk conditions
    • Performs history and physical assessments
    • Performs minor procedures - after established privileges, training and proctoring is completed for the procedures
    • Orders and manages follow up diagnostic testing (laboratory and radiographic findings)
    • Prescribes medications in per standards of care
    • Participates in synchronous and asynchronous telehealth care
    • Identify and assign patient remote physiological monitoring (RPM) point solutions based on HRA and AWV information
    • Monitor RPM data feed, alerts, and alarms to analyze and address clinical interventions based on Telehealth KPIs & patient engagement insights in conjunction with the medical director
    • All aspects of patient management included but not limited to job description
    WHO YOU ARE
    • 2+ years of experience as a FNP or ACNP
    • Current Nurse Practitioner Certification in the State of OH
    • AHA BLS CPR required
    • Current DEA number from the DEA for schedule II-V controlled substances may be required based on State of practice
    • Demonstrated expertise in treating and managing Medicare and Medicare Advantage populations with multiple comorbidities.
