Job summary
The RN Program Coordinator functions as an RN member of the Sleep Healthcare team in a manner consistent with their scope of practice in Pennsylvania and in concert with policies and procedures.The coordinator partners with the Sleep Medicine providers, sleep lab staff and others to coordinate patient care and provide clinical support in clinic and by phone.
Essential Functions:
- Conduct interviews with patient, family and other health care agencies to obtain past medical records, social and medical history and to assess their understanding of the plan of care.
- Educates patient and family regarding all aspects of disease management and diagnostic procedures.
