- The pay range for this position is $55,000

- Working with the older adult population and families through long-term or short-term case management with the goal of supporting successful independence in the community
- Develop individualized care plans for clients
- Home visits and ongoing contact with clients on your caseload
- Master's or bachelor's in social work
- Case Management experience or related work
- Excellent verbal and written skills
