- Build rapport with members through personal experiences to encourage positive behavior changes.
- Coach members on strategies to minimize health risks and connect them to relevant programs.
- Link members to local services and resources and provide necessary support.
- Introduce tools for self-management and self-efficacy.
- Complete all required documentation in a timely manner.
- Drive the medically equipped van occasionally.
- Work closely with a Social Worker to ensure proper care coordination.
- Support the achievement of organizational goals, including housing placement for unsheltered members.
- This role involves direct interaction with patients and may be eligible for additional pay.
- Other duties as assigned.
- School, required plus 3+ years of relevant work experience.
- Bachelor's degree preferred plus 1-2 years of relevant work experience.
- Experience working with homeless populations, mental health, or substance use conditions.
- Ability to work in outdoor environments independently and as part of a team.
- Strong communication and organizational skills.
- Proficiency in operating computers and relevant software.
- Ability to travel frequently with reliable transportation and a valid driver's license.
- Tuberculosis screening policy in effect.
