    Job Title

    Behavioral Health Support Practitioner

    Summary

    Empower adults with mental illness to pursue their goals using the Psychiatric Rehabilitation (Psych Rehab) model. Meet weekly with individuals to identify strengths, needs, and barriers in accomplishing their identified goal.

    Key Responsibilities:

    • Build rapport and effectively engage with individuals.
    • Drive to and meet with individuals in homes and community locations.
    • Complete comprehensive assessments of strengths, needs, and barriers.
    • Develop, implement, update and review treatment plans that are recovery-oriented, strengths-based, and person-centered.
    • Collaborate with individuals on strategies for identifying, achieving, and maintaining goals in four key areas: living, learning, working, and socializing.
    • Write case review documents and facilitate discussion and review of services during staff meetings.
    • Connect individuals to resources and supports in the community.
    • Provide information about mental illness, wellness, and living in recovery.

    Requirements:

    • Bachelor's degree or Certified Peer Specialist (CPS) preferably in a related field such as psychology, social work, human services.
    • Some prior work or internship experience related to mental health services.
    • Valid driver's license with 2+ years of driving experience, reliable vehicle, and car insurance.
    • Willingness to drive throughout the county during the workday to meet with clients.
    • Willingness to transport clients in your vehicle as needed.
