- Hospital: All Saints Spring St/St Francis/Franklin/Elmbrook/St Joseph/Columbia St Marys
- Location: Racine, WI/Milwaukee/Brookfield/Franklin
- Create plan for care across the continuum, integrating patient/family preferences and values.
- Serve as a resource to assess, evaluate, recommend and treat legal, neglect, substance abuse, crisis, psychosocial and various adjustment needs.
- Advocate for resources and removal of barriers.
- Maintain ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of health plan.
- Act as a resource for adequate medical record documentation, complex level of care recommendations, and services as they relate to diagnoses, and treatment options for post-discharge care.
- Registered Certified Registered Nurse credentialed from the Wisconsin Board of Nursing preferred.
- Rehab Therapist specializing in Occupational Therapy (OTR) credentialed from the Wisconsin Medical Examining Board preferred.
- Social Worker credentialed from the state of Wisconsin. Licensed Clinical Social Worker (LCSW) preferred.
- Rehab Therapy Assistant specializing in Occupational Therapy (COTA) credentialed from the Wisconsin Medical Examining Board preferred.
- Associate's Degree in related field required. Bachelor's Degree preferred.
- Minimum of three (3) years clinical experience required.
- Experience in an acute care psychiatry environment and/or equivalent knowledge of disease process is required.

This is a regional PRN position- mainly for All Saints/St Francis/Franklin campuses but also may have opportunity to get hours in Milwaukee area campuses as well ( Elmbrook, St Joesph's, and Columbia St Mary's).
Masters in Social Work and APSW strongly preferred
