- Responds within appropriate time frames to request for spiritual care assessment of patients and families.
- Provides directly or coordinates with community clergy to provide spiritual care to patients and families.
- Documents direct services and ongoing communication with community chaplains and spiritual counselors in the patient electronic record.
- Maintains records of spiritual care services utilization and related activities for quality assurance program development, and policies and procedure review and revision.
- Attends and participates in the interdisciplinary group, IDG, as scheduled, and assists with the completion of the comprehensive Plan of Care.
- Acts as a resource to staff, volunteers, and other clergy, providing inservices, literature and materials to enhance sensitivity to spiritual concerns.
- Provides for funeral or memorial services for patients as requested.
- Participates in volunteer training program.
- Demonstrates job related aspects of patient safety to assure a safe environment, safe and effective use of equipment and technology, and decreased risk of potential adverse patient occurrences.
- Demonstrates knowledge of and supports hospital mission, vision, value statements, standards of care, policies and procedures, operating instructions, confidentiality statements, corporate compliance plan, standards of behavior, and the code of ethical behavior.
- Must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of patients served on his/her assigned unit. Must demonstrated knowledge of principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the unit's and division's policies and procedures.
- Performs other duties as assigned by hospice.
- Possess in-depth knowledge of spiritual issues, particularly as they affect individual and family systems faced with loss.
- Knowledge of and commitment to hospice philosophy of care.
- Understands the spiritual needs of the dying.
- Comfort in an interfaith setting.
- Ability to accept different lifestyles, cultures, beliefs, and values.
- Ability to function effectively as a team member.
- Knowledge of religious community resources.
- Basic computer skills.
- Current Michigan driver's license.
- Access to reliable transportation.
- Ability to obtain CPR certification prior to making solo visits.

- Graduate of accredited seminary or school of theology (BD,MDiv or equivalent theological degree) preferred.
