Case Manager, Discharge Planner - Roanoke, VA

Job summary
The Case Manager Discharge Planner provides discharge planning and assessments, comprised of assessing individuals in the jail setting within 48 hours to determine if individuals would be appropriate for early release,- Education: Bachelor's degree in an acceptable human services field is required or bachelor in other field with minimum 90 hours of human services field classwork and CSAC-Supervisee eligible or Board of Counseling Certified Substance Abuse Counselor (CSAC) eligible.
- Licensure/Certifications: Qualified Mental Health Professional (QMHP) preferred. Substance Use Disorder Case Management (SUD CM): must meet requirements for qualification.
