    Job ID:

    Pay Range: $25-$30/hour[+ Mileage reimbursement]. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

    Note-Location: F 2 F – { Fort Myers (North Fort Myers and Estero are okay too).

    Shift (Start/End Time): Monday to Friday, 8-6,-9-5 EST etc.

    Note: Either LPN or MSW is required, not both.

    Key Responsibilities:

    • Coordinating Care For Special Needs Family On The CMS Plan.
    • Works on non-acute members.
    • Caseload compliance per CMS contract. Field visits, assessments, care plans.
    • Compliance Training.
    • Case Manager, Care Coordinator, School Specialist.

    Key Requirements and Technology Experience:

    • Ped's experience.
    • MSW 1-year peds or LPN with 4-year Peds experience.
    • Bilingual preferred.
    • Experience with electronic medical records.
    • Basic Excel skills.
    • Can use LPN w/4 yrs. Peds experience.
    • Required Certifications: LPN, or MSW without clinical license.
    • Software Skills Required: Microsoft.
