Description

Duties include but are not limited to:

Will utilize all available tools, such as registries, to enable effective and efficient identification and intervention of individual patients and cohorts.

Assures that clinical care provided meets all timelines and required documentation for the Chief of Staff (COS) office.

Participates in multi-disciplinary team meetings to achieve a patient centered, holistic approach for clinical care and needs of the Veteran, focusing on continuous improvement of critical team processes.

Ensures the patient's care plan contains medical recommendations for clinically indicated care.

Provides ongoing, continuous care of one or more assigned panel(s) of Veterans providing health education and health coaching on wellness, disease prevention, chronic care management and self management skills to patients and personal support persons.

Collaborates with informatics technology staff to develop and implement systematized, electronically supported, standardized, tools to support PACT care delivery processes (e.g., pre-visit reminder calls, post hospitalization follow-up calls, recall scheduling procedures, new patient orientation, disease registries and primary care protocols for chronic disease management.

Work Schedule:
Monday- Friday, 8:00am to 4:30 pm (Compressed Tour Available)

Pay:
Competitive salary, annual performance bonus, regular salary increases

Paid Time Off:
49-54 days of annual paid time offer per year (26 days of annual leave, 13 days of sick leave, 10 paid Federal holidays per year and possible 5 day paid absence for CME)

Retirement:
Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA

Insurance:
Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)

Licensure:
1 full and unrestricted license from any US State or territory

CME:
Possible $1,000 per year reimbursement (must be full-time with board certification)

Malpractice:
Free liability protection with tail coverage provided

Contract:
No Physician Employment Contract and no significant restriction on moonlighting

EDRP Authorized:
Former EDRP participants ineligible to apply for incentive.

Education Debt Reduction Program (Student Loan Repayment):

You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment.

Program Approval
, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
