Apply today for our RN Case Manager Rehab PRN opportunity.The Case Manager (CM) ensures high-quality, patient-centered care by managing Rehabilitative care to ensure optimum outcomes. The CM provides, coordinates, and directs care specific to the needs of each Rehab patient. The CM collaborates with the Rehab Program Director and Facility Case Management Director regarding departmental functions. The CM coordinates efforts within the Rehabilitation team.
The Rehabilitation Case Manager is responsible and accountable for coordinating and facilitating successful patient services from pre-admission through discharge.
Coordinates patient, family, physicians and all rehab disciplines to effectively move patient through the continuum of care, including discharge planning.
rehab or case management experience preferred.Must have strong medical knowledge to complete discharge planning assessments, and develop a discharge plan.
