- Must have NJ Home Health Aide Certification
- Ability to travel within the state of New Jersey
- Meet physical requirements: bending, pushing/pulling, standing, lifting up to 50 lbs
- Follow Aide Care Plan. Document all activities using electronic software
- Assist the patient to maintain good personal hygiene, assisting with bathing, skin care, hair, nails, teeth, and mouth, as well as changing bed linens per the care plan
- Help patients get in and out of bed and assist them to ambulate if required by care plan
- Assist patients with the use of durable medical equipment as per the care plan
- Report to the Grace Registered Nurse any change in the patient's medical, nursing, and social needs as related to the plan of care
- Report to the Grace RN any challenges or concerns regarding a patient's home/facility environment
- Plan and prepare nutritious meals in accordance with dietary restrictions and assist in feeding patients when necessary as per the care plan
- Maintain a healthy, safe, and clean environment for the patient
- Perform homemaking duties as assigned
- Assist patients with prescribed exercises as required by the care plan
- Maintain up-to-update clinical knowledge by attending Grace's mandated educational program(s)
- Attend mandatory quarterly aide meetings and other meetings
- Maintain and complete appropriate documentation records daily using electronic software according to Hospice, State, and Federal requirements
- Work according to scheduled hours
- Adhere to the practice of confidentiality with regard to patients, staff, and customers
